Here are your HSC forms. See me if you need your student number.

If you need to colour in a square to indicate that it is your option; click on the square once and then right click it, select ‘format auto shape’, in fill colour select the black colour. Below performance has been filled musicology has not. It is easier to colour in the square rather than put a tick in it.

When you fill in your forms…

• Proof read VERY carefully (particularly anything that I have written). If you find any typos or problems let me know!
• Do not change the formatting of the page. You can not increase the size of a square. If you need to fit in text shrink the text so it takes up less space
• Make sure that you write your student number correctly
• Get your forms done soon/NOW so you can clarify any issues and then it is one less thing to worry about.
• Mrs Necyporuk needs to sign your M8 form so it must be done well before the exam.
• You can print the form and I can photocopy it onto blue paper or I can give you blue paper to print on.

When you complete this form the **** represent specific things that you have done. If you have done a viva for any topic you can try and squeeze it in to the musicology section if you need to. When you mention the pieces you have performed can you mention the name of song and the artist/composer e.g. ‘Bohemian Rhapsody’ by Queen.

The date you put on ALL forms will be the exam date and I won’t know that until 2 weeks or so before the exam. And because I am so obsessive when you do get the date can you write it in this format 12/12/2006
